Non-pharmacological interventions in early schizophrenia: focus on cognitive remediation

چکیده

Objectives There is a growing body of evidence suggesting that barriers to functional recovery are associated with a host of neurocognitive impairments in both the early and later course of schizophrenia. Given the significant influence of cognitive functions on daily functioning, several cognitive training approaches have been developed to improve cognitive deficits in schizophrenia. Increasing amounts of data suggest that cognitive remediation leads to improvements not only in cognition, but also in functional outcomes of schizophrenia. Some researchers speculate that deficits in cognition are more amenable to remediation during earlier phases of the illness, rather than when chronicity has developed. Despite the widely cited benefits of cognitive remediation in long-term course patients, fewer studies have examined the extent to which cognitive remediation has practical implications in the early stages of schizophrenia. The aim of this paper is to review the available literature on cognitive remediation in the prodromal phase and the early course of schizophrenia.

Reward learning as a potential target for pharmacological augmentation of cognitive remediation for schizophrenia: a roadmap for preclinical development

RATIONALE Impaired cognitive abilities are a key characteristic of schizophrenia. Although currently approved pharmacological treatments have demonstrated efficacy for positive symptoms, to date no pharmacological treatments successfully reverse cognitive dysfunction in these patients. Cognitively-based interventions such as cognitive remediation (CR) and other psychosocial interventions howeve...

Pharmacological Augmentation of Psychosocial and Remediation Training Efforts in Schizophrenia

Pharmacological approaches to cognitive enhancement have received considerable attention but have not had considerable success in improving their cognitive and functional targets. Other intervention strategies, such as cognitive remediation therapy (CRT), have been shown to enhance cognitive performance but have not been found to improve functional outcomes without additional psychosocial inter...

Cognitive Remediation in Schizophrenia

Cognitive deficits in schizophrenia are pervasive, severe, and largely independent of the positive and negative symptoms of the illness. These deficits are increasingly considered to be core features of schizophrenia with evidence that the extent of cognitive impairment is the most salient predictor of daily functioning. Unfortunately, current schizophrenia treatment has been limited in address...

Does practice make perfect? Results from a Chinese feasibility study of cognitive remediation in schizophrenia.

Patients with schizophrenia often receive little by way of non-pharmacological interventions. Despite this, promising outcomes in programmes targeting cognitive deficits have been reported, suggesting that this is an area worthy of further investigation. The aim of the study was to implement and evaluate a brief computerised cognitive remediation programme designed to improve memory and attenti...

Factors affecting cognitive remediation response in schizophrenia: the role of COMT gene and antipsychotic treatment.

Cognitive remediation is the best available tool to treat cognitive deficits in schizophrenia and has evidence of biological validity; however results are still heterogeneous and significant predictors are lacking. Previous studies showed that cognitive remediation is able to induce changes in PFC function and dopaminergic transmission and thus the study of possible sources of variability at th...
